Our services support your client and the Caregiver of a person with a disability or other conditions by connecting them with community resources and other support as needed.
ConnectAbility serves Bexar County and surrounding areas. We are committed to identifying the best, most accessible resources for people with disabilities and other conditions so that they can maintain maximum wellness — in both mind and body.
Our services are offered at no cost, regardless of income or insurance status.
Care Coordination
Our Care Coordinators will help the Caregiver of a person with a disability to connect with community resources for which they are eligible and entitled based on their individual needs. Additionally, our Care Coordinator will act as an advocate and provide guidance and other resources as needed.
We will assist your client's family member or caregiver with educational activities, support groups, and other resources to ensure the caregiver remains healthy — both emotionally and physically — to assist with your client's journey.
We help your clients connect with others who have shared a similar experience. A person who has sustained a life-altering injury or disability similar to your clients will assist with guidance, support, and education.
We partner with local organizations to provide a variety of events for individuals with life-altering disabilities, as well as their families and caregivers.
