A life-altering injury can be devastating, but it does not have to be life-ending.

We are here to help clients, their families, and caregivers find the strength, education, and support they need to live to maintain wellness in mind, body, and spirit. 

We will find the services that fit your needs.

All of the services we offer work comprehensively and holistically, addressing a multitude of issues depending on the needs of the affected individual and their family.

Our services are offered at no cost, regardless of income or insurance status.

Care Coordination

Our Care Coordinators will help you connect with community resources based on your needs. Additionally, your Care Coordinator will act as an advocate and provide guidance and other resources as needed.

Common Questions

ConnectAbility serves Bexar County and surrounding areas. Currently, we are providing services for people with:

  • Traumatic Brain Injury
  • Spinal Cord Injury
  • Amputation
  • Stroke
  • and other related injuries conditions

All of our groups are offered to people with disabilities and their loved ones/caregivers.

Care coordination is provided in Bexar and surrounding counties, but anyone may attend support groups or access peer mentoring in person or virtually.

Family + Caregiver Support

ConnectAbility staff will assist a family member or caregiver with workshops and other resources to ensure the Caregiver remains healthy, both emotionally and physically, to assist their loved one on their journey.

Anyone who offers care to a loved one or an individual with a disability.

Your health and well-being is of equal importance than that of your loved one.

Peer Mentoring

Connect with others who have shared your journey. A person who has sustained a life-altering injury similar to yours will assist with guidance, support, and education.

Common Questions

Our peer mentoring coordinator will meet with you to discuss your needs and goals.

You will then be matched with someone who can offer the best guidance for you.

  • Attend necessary training
  • Allow for a criminal background check
  • Agree to meet 4-6 hours monthly with your mentor
  • Provide progress notes, as needed

We partner with local organizations to provide a variety of events for individuals with life-altering injuries as well as families and caregivers.
